Do you have Cookies enabled? I've found that you normally have to have them
enabled for sign-up forms. That's why I like GuideScope. It lets you selectively
enable/disable cookies based on the sender.

I'm unable to reproduce this.
Form is a bit clearer now - but as far as I can tell (Using NS4 and IE6 as
test clients) everything works like its supposed to.
And, does anyone have an e-mail address for the
