IIRC the 16x64 will mean that this is a 128Mb DIMM.  (16Mb x 64 bits wide/ 
8 bits per byte)

Likely a PC-100 DIMM, from the PC100 part.

It is a 168-pin DIMM.

I has a total of 16 chips on it (8 in front, 8 in back)
There is a stick-on tag that reads:

16X64SYPC100                882010
        B A R   C O D E
Warranty VOID if Removed
12/99                    0773 00011

On the back (center) if reads:

M500PCG REV A
PCSDRAM REV 1.0

I don't see a name anywhere.
