I have the same tape as well. Havent tried it out yet though.

How many people (who were adults by the time the PC came out), DON'T have
one of those tapes?
I don't and I ran a whole lab full of original IBM PCs when they were new
but I never saw one of the tapes.

Did you buy them with or without disk drives?
Did you buy directly from IBM, or from a vendor who supplied them already
unpacked?
Did you get the "Guide to Operations" book for each machine?
All of the 16K 5150s that we bought (without disk drives) had one of the
tapes inside the "Guide to Operations".
Maybe they stopped with the 256K motherboard models?
Unfortunately, a few years ago, a college employee unilaterally discarded
all of the old materials; otherwise I could remedy the situation.
