related in Reader's Digest Humor in Uniform section: An Army cook in Vietnam got an

eyeball so he had to see the unit medic to get it removed. Some time later, he was
notified that he was receiving a Purple Heart. The reason cited was "shell fragment
in eye." Apparently, someone was just glancing through through the undoubtedly
numerous medical treatment records to discern who to present the medals to and found his
record with its deceptive entry.
