I have been using E-Teknet in Arizona for some time. They are not the cheapest, but they are very good. The match your board characteristics with the best shop in China to make it. I've never had a bad board from them, and I have done everything from two-layer boards smaller than a postage stamp to 6-layer boards the size of a full sheet of paper. If you order 4 or less they don't charge you for the testing. Jon
