Amazingly I've found it. The label on the bottom says :
IBM
MOD 5277-1
The 5277 mouse is used with the 5271 PC/XT (or 5273 AT) 3270/G and
3270/GX. I believe it may connect to the top DE9 port present on some
models of the 3270 PC keyboard interface adapter.
