ViewMate from Pentalogix will do it for $49. It is the best gerber editing program I
have ever used.
It will open a bitmap and generate a monster gerber file. You will have to manually add
the holes. From looking at the Pentalogix web site, it looks like
the application is now called ViewMasterEZ
(http://www.pentalogix.com/Products/ViewMasterEZ/). They still have a ViewMate app, but
it is demo-ware that can't save any work.
