Actually, yes.
As someone else who writes ~70-column text: no, not every line break.
I insert some of them manually. The rest are created automatically by
editor autowrap.

people ever change their mind and have to insert/delete text?
Then what, rejigger all the lines by hand? Surely you don't still
do that?
Actually, yes.
I do or don't, depending on how automated a process your "by hand"
includes. If I need/want to reflow a paragraph, I manually delete each
line break and manually provoke editor autowrap to get it reinserted it
at the then-correct spot. (For my value of "correct", of course.)
For example, my first paragraph here initially read "...text: no, not
manually. I insert some of...". I later changed it and then rewrapped
the rest of the paragraph this way, with ^E space ^D ^E space ^D ^E.
I very occasionally will manually force linebreaks to go places other
than where editor autowrap puts them. (I'm talking about running text
here; things like quotes of computer output also have hard line breaks
but are more often inserted by copy-&-paste or by having the editor
insert a file's contents rather than by my typing them.)
If I were less lazy, I'd instead generate multipart/mixed text with the
running text marked format=flowed and quoted output and other such
non-reflowable text not so marked. So far doing that hasn't been worth
it to me.
Of course, some people may find that laziness, or its product,
reprehensible, as I do paragraph-length lines; I invite anyone who
feels that way to procmail me to /dev/null (or killfile me, or whatever
term), or just ignore my mail the way I sometimes skip most of a
long-lines message after a glance at its beginning (as I did with the
message that started this subthread off - it was difficult enough to
read, and the first half-line or so sounded uninteresting enough, that
I didn't bother reading the rest).
And I'm not kidding.
