The Wikipedia entry on FAT and related topics are very extensively OCD on such topics even
by the usual Wikipedia standards. I recommend them as you'll probably find them a good
read.
I waded through most of it as part of my research into writing filesystem drivers, but
haven't yet used the information in anger. So I can only confirm that it is definitely
extensive but not that it is definitely correct :)
