I bought one of these last year thanks to Roy's suggestion. It is a
Phoenix CPS-10 from Phoenix Electronics Inc. of Clay Center, KS. I
got mine off of eBay, and it wasn't cheap, I think I paid about $60
for it.
Of course a couple days later I lucked out and was given a C-128
powersupply of similar construction that appears to have been
modified to also run a +4. I still need to check the pin-out on the
power supply, but it seems likely given the number of +4's included
with the C128.
