Unlike later "winmodems", these modems present themselves as serial ports
(ISA, basically) to the OS, so you should only need card services software
for the OS them to function (included with Win95). There /might/ also be a
plug-n-play descriptor file somewhere for an OS such as Win95, but you may be
able to get the modem to work without it (or with the defaults Win95 wants to
use). For something like MS-DOS, the only thing you would need for these
modems is the PCMCIA card services software (in this case, from Toshiba).
There are also a lot of USR files in
ftp://ftp.usr.com/ and there may be
something in there for your modem, but it isn't the most organized of FTP
servers.
I used 100s of these very modems in remote-embedded applications over the
years. They were among the easiest modems to configure and were a heck of a
lot less expensive to pair up with a PCMCIA (16-bit) to PC/104 carrier board
than an actual PC/104 modem. PCMCIA modems also had the additional benefit of
being easier to replace in the event of storm damage, too (even with a nut
driver, disassembling PC/104 stacks is a hassle).
I failed to notice that this PCMCIA card is a combo adapter with both an
ethernet NIC and modem. This board probably won't be found on USR's site
because of the way these companies merged and split. I had the same issue
trying to track down drivers for another USR/3Com combo product many years
ago (and never could find anything).
Megahertz was purchased (or merged with, depending on whom you ask) by US
Robotics in 1995. US Robotics was then acquired by 3Com in 1997. In 2000,
NatSteel and Accton formed a joint company, resurrected the US Robotics
brand and took over 3Com's modem business. Solectron bought NatSteel in
2001 and in 2004 sold their stake in US Robotics to USR's management. In
2010, HP (sadly) bought what was left of 3Com.
Modem-only devices went with the "new" US Robotics company in 2000, but
combo NIC devices went with 3Com (now HP). The drivers you need would have
been on

ftp://ftp.3com.com/pub/nic/XJEM3288/XJEM3288.EXE],
which appears to have been shut down after the HP acquisition.

If someone has any contacts at HP, this might be another thing to point
out and see about getting them to fix. The 3Com ftp files are almost
certainly still available internally at HP, but they do not appear to be
available via any of HP's public ftp servers. I ran into this with some of
the Compaq stuff in the past, too.
