I believe the name of the tool was "mkimage". I took a quick look on their
site but couldn't find it. All I could find for download was "mkdisk" but
this does not seem to be able to extract from and ods-2 cd, it just looks
like it creates an empty disk image for use by the emulator.

gone from CD-ROM to running VAX under Windows? How did
you get files off the distro?
Take a look around the Charon-VAX site, they're back to offering a
Hobbyist
version I hear, and when they first released their emulator you could
download a tool to do this.
