What about a broken copper trace, defective data or address buffer/driver
or mux/demux? I had a similar fault on the graphics adapter in the IBM
5110, one bit of a multiplexer was bad.

Hi
It could also be a problem in the address decoder. I've also seen
interaction with another RAM on the same line as causing such
problems as well.
In the case of another RAM, it is ususally the previous RAM
accessed. It tends to stay selected a little longer than it should.
My bet is on the address decoder not selecting that RAM.
