Here's one that I whipped up several years ago. As I remember, it
worked well enough to boot RT11 from TU58, but it hasn't been too
extensively tested and, of course, I don't make any guarantees.
Also, I hand-assembled this so it may well be some strange mix
of DEC and UNIX assembler notations. It should be relocatable,
but my notes suggest I loaded it at 1000.
