Yes, it starts at 3, providing you acknowledge that "Sector 1" is a
completely artificial notion. (i.e. There are a few hard-sectored
systems that don't look for the index pulse at all.)
By that I assume you mean that they use the index pulse to mark the sectors,
but ignore the track start marker?
Otherwise how could they be called hard-sectored if they weren't synchronising
the sector boundaries against sector marks on the disc?
