Duh, I turned on my brain and buzzed out the connector to the PC board.
If I number the connectors like this

| 1 2 3 |
+-------+
| 4 5 |
+-------+
Then 2, 4, 5 = GND, 3 = +5V regulated, 1 is > +12V unregulated.
That is, pin 1 goes to a 7812 regulator, pin 3 goes directly to +5 on
the digital logic, and the other three are grounds.
I've gotten so used to quickly finding things with google I forgot some
of the old fashioned techniques -- like figuring it out myself.
Jim, what make/model of 8" drive are you using? The most usual DC
power connection to one looks like the 'sketch' below. The AC
power to the motor is a separate connector.
