It might have originally been in the inside jacket of on of the
IBM manuals.
I don't have any way to digitize it.
Guide To Operations on some.
On the DISK diagnostics, the
"DIAGNOSTIC" disk came in the Guide To Operations" book.
"ADVANCED DIAGNOSTICS" was NOT AVAILABLE to the public, but was necessary
for hard disk low-level formatting.
Once again, third party products were essential.
