Scan the form, load it into your favorite Word
Processor or Image Editor
as a background image, and type over the top of it. You don't need to worry
1) I don;t have a scanner
2) Muy favourite word processor is called TeX....
3) If the form has a coloured background or logo, I'd have to buy a
coloured printer.
The manual typewriter is a lot simpler and easier to keep running...
