I think the issue is that the Teledisk images were built including
some additional sectors that are essentially garbage and could be
ignored.
My solution at the time after spending some time looking at this was
to rebuild TD02IMD from the source code available on Dave's site to
ignore these extra garbage sectors that were cause the fatal errors.
I'd have to look back at my notes on this to remember exactly what I
did here.
It would be a useful project to go through the Teledisk images on the
HP Museum site and convert them all to ImageDisk format.

Have you tried using Dave's TD02IMD program to directly convert the
Teledisk .TD0 to .IMD files?
Sorry I forgot to mention that I tried that and it failed with an error
about different sector sizes on the same track.
