Unlike later "winmodems", these modems present themselves as serial ports
(ISA, basically) to the OS, so you should only need card services software
for the OS them to function (included with Win95). There /might/ also be a
plug-n-play descriptor file somewhere for an OS such as Win95, but you may
be able to get the modem to work without it (or with the defaults Win95
wants to use). For something like MS-DOS, the only thing you would need
for these modems is the PCMCIA card services software (in this case, from
Toshiba).
There are also a lot of USR files in
ftp://ftp.usr.com/ and there may be
something in there for your modem, but it isn't the most organized of FTP
servers.
I used 100s of these very modems in remote-embedded applications over the
years. They were among the easiest modems to configure and were a heck of
a lot less expensive to pair up with a PCMCIA (16-bit) to PC/104 carrier
board than an actual PC/104 modem. PCMCIA modems also had the additional
benefit of being easier to replace in the event of storm damage, too (even
with a nut driver, disassembling PC/104 stacks is a hassle).
