Charles, is it called "Core Memory Reporter Notebook"?  If so, you can get 
it for $2.21 here:

That website is my go-to place when I'm looking for books that are WAY 
over priced on Amazon.  I recently bought a copy of the 6th edition of 
Charles Petzold's Programming Windows from there for $17. :)
